Invasive Lobular Breast Cancer

About 1 in 10 breast cancers diagnosed (10%) are invasive lobular carcinoma. This means that the cancer is growing in the cells that line the lobules of the breast. Invasive lobular cancer can develop in women of any age. But is most common in women between 45 and 55 years old. It is possible for men to get invasive lobular breast cancer, but this is very rare.

The types of cancer vary in their prognosis and the way in which they present. Lobular carcinoma (in situ and invasive) will be discussed here. The term ‘in situ’ refers to pre-invasive breast cancer. This is breast cancer which has not yet penetrated (’invaded’) through the basement membrane (the membrane at the base of the epithelial lining of ducts or glands). In situ carcinoma has the potential to become invasive carcinoma, and so is treated as an early form of breast cancer. In the left-hand image below, you can see how the normal cells which form the lobules of the breast might look. In lobular carcinoma in situ (LCIS, the image to the right), the lobular cells have developed the ability to multiply out of control - one of the characteristics of cancer. The cancerous cells have not yet spread beyond the lining of the lobule. This is known as LCIS.

Despite the fact that its name includes the term “carcinoma,” LCIS is not a true breast cancer. Rather, LCIS is an indication that a person is at higher-than-average risk for getting breast cancer at some point in the future. For this reason, some experts prefer the term “lobular neoplasia” instead of “lobular carcinoma.” A neoplasia is a collection of abnormal cells.

As for most types of breast cancer, surgery will be the first treatment. This may be a mastectomy or wide local excision. You will usually be given a choice between these two types of surgery. If a wide local excision is possible there is a chance that a second operation may be necessary to make sure that a clear enough area of tissue around the lump is taken. In some cases, to get a clear area of tissue it may be necessary for the whole breast to be removed. Invasive lobular cancer can sometimes affect more than one area within the breast. If this is the case the surgeon may recommend a mastectomy.

LCIS is viewed as an uncommon condition, but we don’t know exactly how many people are affected. That’s because LCIS does not cause symptoms and usually does not show up on a mammogram. It tends to be diagnosed as a result of a biopsy performed on the breast for some other reason.

If you have a wide local excision you will usually be offered radiotherapy to reduce the risk of the cancer coming back in the same breast. If you have a mastectomy you may not need radiotherapy.

Inflammatory Breast Cancer

Inflammatory breast cancer has no lump or mass that can be felt. It is a very rare and dangerous type of breast cancer. It cannot be detected by self breast examination and mammogram. In inflammatory breast cancer there is infiltration of the skin and lymph vessels of the breast by cancer cells. The symptoms of inflammatory breast cancer are a swollen, red and warm breast which is brought on by the lymph vessels becoming blocked by the breast cancer cells. The breast has a typical appearance of an individual with cellulite. Other symptoms of inflammatory breast cancer include, on the affected side, enlarged lymph nodes present under the arm or it may be above the collar bone.

An asymptomatic breast cancer patient was breast-feeding and developed what her doctor thought was lactational mastitis or inflammation of the breast brought about by breast-feeding. It never cleared up and did not hurt much - there was no fever sign of infection. It hadn’t gone away or gotten worse in six months. Another patient, not breast-feeding, noticed that one breast had suddenly become larger than the other; there was also redness and swelling. In both cases, the doctors at first thought the women had breast infections. It is very easy to mix up the symptoms of inflammatory breast cancer with those of a breast infection. If you become aware of any of the signs of inflammatory breast cancer, see your doctor immediately.

The surgeon may think it is best if the breast is removed and sometimes the breast is removed too early. This early removal of the breast increases the risk of the disease making reappearance.

As with all advanced cancers, chemotherapy is started with three or four cycles of Adriamycin and Cytoxan with or without Taxol or Taxotere. After which local treatment can be done - usually in the form of mastectomy. After mastectomy, most women will receive four more cycles of chemotherapy followed by radiation therapy to the chest wall. Serious though it can be, inflammatory breast cancer is still an extremely variable disease.

These treatments have greatly improved the prognosis for a woman with inflammatory breast cancer. Recent studies have shown as much as a 50% survival rate after 5 years and a 35% survival rate after 10 years.

When breast conservation therapy is used as treatment for the cancer, there is a risk of cancer redeveloping in the same breast.

Some hormonal factors increase the risk of breast cancer. A woman who started her menstrual period before the age of 12 has a slightly increased risk of developing breast cancer. This is also true of women that have menopause after 55, women who have a first child after age 30, and women that have never had children.

Do a mammogram after age of 40. Women whose mammograms show many dense areas of tissue have an increased risk of breast cancer compared to women whose mammograms reveal mainly fat tissue. A woman who is told that her mammogram has areas of increased density should ask her health care provider to explain what this means.
